Care Coordination
For: Seniors, their adult children, families and caregivers.
Service description: Once a comprehensive needs assessment has been completed and the specific care, services and programs that would most benefit the senior have been identified, a service plan is created and monitored by JSSA professionals on an ongoing basis. Monitoring ensures that needs are met; resources are identified; services are accessed and delivered in an integrated manner and in accordance with the plan to meet specific needs. The plan is updated and modified as appropriate.
Service providers: Clinical social workers, geriatric case managers, and nurses.
