Service description: A comprehensive assessment determines particular areas of need, the type of services and programs that might be most appropriate and the individualized action plan to be implemented to ensure the best possible outcomes.
Recommended services could include individual, group or family workshops, a psychiatric evaluation and/or medication management plan, educational or neuropsychological testing and consultation, specific support groups, or links to helpful community resources.
Service providers: Clinical social workers, case managers, psychologists and psychiatrists.
